SUPPLEMENTAL HEALTH AND PROTECTION BENEFITS

Even with comprehensive coverage from your medical plan or life insurance, you may still have out-of-pocket expenses if you become critically ill, seriously injured, need to be hospitalized or require long-term care. These benefits are designed to fulfill a variety of needs, including supplementing your existing medical coverage to give you additional financial support when something unexpected happens or to prepare for long-term care for your loved ones. Life is unpredictable, and having these supplementary benefits can help you be prepared no matter what happens.

We offer critical illness, accident and hospital indemnity insurance administered by Securian, as well as term life insurance with long-term care administered by Trustmark.

CRITICAL ILLNESS INSURANCE

Critical illness insurance pays a one-time, lump-sum, cash benefit if you are diagnosed with a covered critical illness or condition, such as cancer, heart disease, stroke or other serious condition. You can spend the benefit to cover medical costs, groceries and any other living expenses.

You can elect to purchase from $5,000 to $50,000 in critical illness insurance for yourself, as well as 50% of your benefit for your spouse and each child, and 10% of your benefit for dependent parents. Coverage includes:*

  • Cancer diagnosis
  • Reoccurrence coverage for some cancers, burns, heart attack and stroke
  • Services and surgeries, such as organ transplant, end-stage kidney failure, coronary artery bypass surgery, loss of sight, speech or hearing, coma, etc.
  • Diseases, such as Alzheimer’s, multiple sclerosis and rabies
  • Infections, such as tetanus
  • Childhood conditions, such as cerebral palsy, cleft lip, cystic fibrosis and diabetes type 1

*Initial diagnosis must occur on or after the policy effective date.

Infertility Benefits

When you enroll in critical illness insurance, infertility treatment to improve fertility is considered a related covered benefit and certain medications and procedures may be covered. View details about infertility treatment in the critical illness certificate of insurance.

Health and Wellness Benefit

When you enroll in critical illness insurance, you can earn an additional cash benefit just for taking care of your health. The Health and Wellness Benefit of $75 is paid to you, your covered spouse, and/or your covered children by Securian when you complete one of the health and wellness activities listed in the health and wellness screening flyer. (To be eligible, you must receive these services when not in a hospital on an inpatient basis.)

ACCIDENT INSURANCE

Accident insurance pays you cash when you are injured in a covered accident. You can spend the money to cover medical costs, physical therapy, rent/mortgage or any other living expenses.

You can elect to purchase coverage for you, your spouse and/or your child(ren). Here are some common services and the cash benefit associated:

  • X-ray: Up to $175
  • Fractures and broken bones: $1,000 up to $10,000 (includes but not limited to fractures of the ankle, collarbone, foot, hand, hip/thigh, nose, pelvis, wrist)
  • Urgent care visit: Up to $300 per visit
  • Emergency room visit: Up to $300 per incident
  • Major diagnostic exam: Up to $300
  • Accident follow-up: Costs vary
  • Out-patient rehabilitative therapy, such as physical therapy: Up to $600

Health and Wellness Benefit

When you enroll in accident insurance, you can earn an additional cash benefit just for taking care of your health. The Health and Wellness Benefit of $75 is paid to you, your covered spouse, and/or your covered children by Securian when you complete one of the health and wellness activities listed in the health and wellness screening flyer. (To be eligible, you must receive these services when not in a hospital on an inpatient basis.)

HOSPITAL INDEMNITY INSURANCE

Hospital indemnity insurance pays a lump-sum, cash benefit when you need to go to the hospital and can be ideal for labor and delivery or other planned hospitalizations. You can spend the benefit to cover medical costs, child care, pregnancy-related expenses and any other living expenses.

You can elect to purchase coverage for you, your spouse and/or your children. Here are some common hospitalization services and the cash benefit associated:

  • Hospital admission: Up to $1,500 for the initial hospital stay
  • Daily hospital limit: Up to $150 for up to 30 days
  • Additional coverage, such as for mental illness, substance use, etc.

This coverage also includes Benefit Bump – the “easy button” to help growing families navigate their benefits, time off and wellbeing.

Health and Wellness Benefit

When you enroll in hospital indemnity insurance, you can earn an additional cash benefit just for taking care of your health. TERM LIFE INSURANCE WITH LONG-TERM CARE BENEFITS

Long-term care benefits can help protect your retirement savings, ease the burden of caregiving by your loved ones, and allow you to choose the setting in which you receive care. Coverage is provided by Trustmark.

As life insurance, this benefit protects your family with money they can use as they like. For example, they can use it to pay the mortgage or rent, education for children or grandchildren, retirement expenses, family debt or however your family members choose. It also provides benefits while you are living if you need home health care, assisted living or nursing care.

You can elect to enroll in this insurance during annual enrollment. You can elect up to $150,000 (eligible up to age 70) of coverage for yourself without answering any medical questions. If you elect coverage for yourself, you have an additional option to elect up to $30,000 for your spouse with only one medical question.
